Also on this certificate, her birthplace is listed as Floyd County, her birth date as December 13, 1877, her marital status as widowed, her parents as Wash Leake and Rachel Music, the informant as Dealpha Rice, and the burial place as the family cemetery in East Point, Kentucky.

He was born June 27, 1895 in Jetts Creek, Breathitt County, Kentucky, died December 22, 1983 in Owensboro, Daviess County, Kentucky, and is buried in Rosehill Elmwood Cemetery in Owensboro, Daviess County, Kentucky.

Notes: When Albert McIntosh's World War I draft registration card was filled out on June 5, 1917 in Breathitt County, Kentucky, he stated that he was a self-employed farmer in War Creek, Kentucky, that he had a mother and wife who were dependents, and that he was born June 27, 1895 in Jetts Creek, Kentucky. He was found to be short and stout, and he had gray eyes and dark brown hair.

Information found on findagrave.com and a photo of their headstone shows that Lucy B. McIntosh lived from May 18, 1906 to July 22, 1988, and was buried in Rosehill Elmwood Cemetery in Owensboro, Daviess County, Kentucky, and that Albert McIntosh lived from June 27, 1895 to December 22, 1983, and was buried in the same cemetery. It also shows that they were married November 3, 1927.

According to Social Security death record transcripts, Lucy B. McIntosh was born May 18, 1906, and died July 22, 1988. She received her Social Security card prior to 1951 in Kentucky, and her last place of residence was in Lexington, Fayette County, Kentucky.

She was born April 6, 1890 in Kentucky, died January 12, 1974 in Kettering, Montgomery County, Ohio, and is buried in Bellbrook Cemetery in Bellbrook, Greene County, Ohio.

Notes: Bertha Rice was the sister of Bessie Rice.

According to her obituary, Jane Cunningham Rice (age 96), daughter of the late Timothy and Elizabeth Spears Cunningham, was born July 29, 1864 in Paintsville, Kentucky, died November 10, 1960 in Guthrie Nursing Home in Louisville, Kentucky, and was buried in Mayo Cemetery in Paintsville. She was preceded in death by her husband, Greenville Rice, in 1904.
